Sports physicals: must be updated in order to try out and practice for any sport. Students' sports physicals will expire 14 months from the date signed by the physician.
*Note - A sports physical is different from a regular physical from the student's doctor.
This Sports Physical is needed for 5th, 7th, and 8th grade and those younger cheerleaders as well.
- 6th graders are able to use their regular physical.
Concussion form and Code of Conduct forms: All student athletes must have these forms signed by both the athlete and parent before participating in a sport. Student athletes sign these forms once per school year, not for each activity.
