Dear Parents and Guardians,

We hope your family is doing well and staying safe during the pandemic.

We are NOW registering for the 2021-2022 school year.

If you have a child who will be entering Kindergarten or 6th gradein the 2021-2022 school year, you should have received a notice from the school nurse with important information regarding health requirements

Children entering Kindergarten require:
  • A school physical (on the revised 11/2015).
  • 4 year old booster immunization
  • Proof of Lead screening
------------------------------------------------------------------------------------------
Children entering 6th grade require:

  • The "Miles of Smiles" dental permission slip is available on the district website under the "Required Health Forms" tab in English and Spanish. (here)
------------------------------------------------------------------------------------------
Students in Grades K, 2nd and 6th require the nurses approval before registration is approved for the 2021-2022 school year.
------------------------------------------------------------------------------------------

  • Students will be excluded from school if they do NOT have these requirements on file by the first day of school.
  • Doctors tend to be very busy in the summer, and so we urge you to make the necessary appointments for required physical and immunizations now so that you can be sure to have everything in order before the new school year begins.

Completed forms can be brought directly to the student’s school starting on May 13th or during the summer break, during business hours at the District office located at 1910 W 170th Street. Please drop off documents in an envelope with the student’s name, school and parent contact number for any additional needs or questions.

Make a copy to retain for your own records.
  • Forms can uploaded in parent portal, they can also be scanned and sent via email [email protected]
  • If you have any questions, please first contact the nurse at your child’s school, or if she is not available, contact Yolanda Payne, Assistant Superintendent @ 708-825-0067
