Welcomes Returning Students
2023-2024
Dear Parents and Guardians,
We hope your family is doing well and staying safe during the pandemic.
We are NOW registering for the 2023-2024 school year.
If you have a child who will be entering Kindergarten or 6th grade, in the 2023-2024 school year, you should have received a notice from the school nurse with important information regarding health requirements.

Children entering Kindergarten require:
- A school physical (on the revised 11/2015).
- 4-year-old booster immunization
- Proof of Lead screening if not completed.

Children entering 6th grade require:
- A school physical (on the revised 11/2015).
- Tdap
- MCV4 vaccination required on or after the child’s 11th birthday, NO EXCEPTIONS per the state.

Dental Exams are mandatory per the Illinois State Board for Kindergarten, Grade 2, and Grade 6.
"Miles of Smiles" provides Free Dental exams to our students with permission from the parent/guardian.
- The "Miles of Smiles" dental permission slip is available in both English and Spanish. (here)

Students in Grades K, 2nd and 6th require the nurses approval before registration is approved for the 2023-2024 school year.
- Students will be excluded from school if they do NOT have these requirements on file by the first day of school.
- Doctors tend to be very busy in the summer, and so we urge you to make the necessary appointments for required physical and immunizations now so that you can be sure to have everything in order before the new school year begins.

Completed forms can be brought directly to the student’s school during business hours:

Please drop off documents in an envelope (for the school nurse) with the student’s name, school and parent contact number for any additional needs or questions.
