THINGS TO KNOW IN ORDER TO COMPLETE THE REGISTRATION PROCESS
- STEP ONE - ONLINE
- The pre-registration process online MUST be completed before completing the FULL in-person registration process. The pre-registration process online requires parents/ guardians to:
- Complete all fields on the Enrollment Form online and upload documents (see link below)
- Complete the Household & Income Form (complete IN ADVANCE) and bring to the in-person registration.

- STEP TWO - IN-PERSON
- Once all fields on the Enrollment Form are complete, parents/ guardians have to bring the following documents in-person (see below) to be reviewed & verified by school personnel. The in-person registration dates/ times are on the website.

KNOWINGLY ENROLLING OR TRYING TO ENROLL A NONRESIDENT (OUT OF DISTRICT) STUDENT WOULD BE COMMITTING A CLASS C MISDEMEANOR, WHICH COULD BRING A 30 DAY JAIL TERM AND A $1,500 FINE.
IF YOU ARE FOUND TO RESIDE OUTSIDE OF DISTRICT BOUNDARIES, YOU WILL BE REQUIRED TO PAY $22,472 IN YEARLY TUITION.
Important Notes:
- For students grades K - 8th, Hazel Crest School District 152 ½ DOES NOT accept out of district students. All students in District 152 ½ are required to verify that they reside within the boundaries of the school district. Residency is established where the family legally lives. A person cannot have permanent residence in two places at the same time.
- Guardianship papers are not sufficient evidence that a student is a legal resident in the District. The applicant must prove that the guardian has full legal custody and control of the student for an indefinite period of time.
Families must FULLY complete the registration process in order for your child to attend the first day of school on Tuesday, August 19, 2025.
