Required Registration Documents

Parent Quick Checklist

 

ALL PARENTS / GUARDIANS MUST HAVE THE FOLLOWING:

 

- Original Birth Certificate (new and returning students

- Current Illinois State Driver’s License or State ID with a valid address within School

- District 152 ½ boundaries. The ID must match the residency address. - Two (2) current utility bills (water, gas, electric) Must have name, address, and current date. **Not older than 60 days. 

- Updated Medical, Immunization records, and physical (based on grade requirements) 

- Household & Income Form (complete IN ADVANCE)

If applicable…

- Utility Inclusion Verification Letter - Certified of the Minor Guardianship Order (if applicable)

- Current 906 Placement Form from the Department of Children & Family Services (if applicable for children in care)

- Sports physical (optional)

  **Documents not allowed: lease or mortgage documents, phone bill, cable bill, monthly car payment, bank statements, insurance documents, library cards

 

ADDITIONAL DOCUMENTS FOR NEW STUDENT:

 

- ISBE Transfer Form from previous school attended

- Report cards from previous school attended

- Current Medical, Immunization records, and physical **Not older than 1 year

- Proof of Income (PreK only)**

- Current IEP and 504 (if applicable)

**All PreK students must complete the screening process before registration
