MEDICAL - ILLINOIS SCHOOL REGISTRATION REQUIREMENTS


 

Medical Compliance: 

  • All health records must be on an Illinois approved form. 

  • Required health forms including Physical examination Health Certificate (complete health history sign and date) including allergies, medication taken at home and school that are prescribed by the doctor with supporting documentation, immunization records, dental exam, eye exam, and sports physical.

 

Parents: 

  • PLEASE THOROUGHLY complete the Medical Tab on the Enrollment form online

  • Please make sure the correct grade has been marked. 

  • Please upload clear medical documents in the Enrollment form online including: seizure action plan, asthma action plan, diabetes action plan, dietary restriction forms. 

  • All medication forms have to be renewed each school year.


 

IMMUNIZATION & HEALTH INFORMATION REQUIRED PER GRADE LEVEL

PreK

Physical Examination: CURRENT FORM REQUIRED.

Immunizations:

  • (DTP/DTaP): Three doses by age 1, one additional dose by age 2.

  • Polio(3), Measles, Mumps, Rubella (MMR)(1), Pneumococcal(4) Hepatitis B(3), Varicella (Chickenpox)(1), Haemophilus Influenza Type B (HIB)(4): Age-appropriate doses.

Lead Screening: Is required for your child one time prior to entering kindergarten for children 6 years or younger. There is a portion on the Physical Examination that needs to be completed to document this requirement. Required for children up to age 6.







 

K

Physical Examination: CURRENT FORM REQUIRED.

Immunizations:

  • DTP/DTaP: Four or more doses, with the last dose after the 4th birthday.

  • Polio: Four doses, with the last dose after the 4th birthday.

  • MMR: Two doses.

  • Varicella: Two doses.

Eye Examination: Required for all children entering kindergarten or enrolling in an Illinois school for the first time. Must be completed by a licensed optometrist or medical doctor within one year prior to the first day of school.

Dental Examination: Must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.

1st - 5th

Physical Examination: Required for first-time enrollment in Illinois schools.

  • DTP/DTaP: Booster dose after the 4th birthday.

  • Polio, MMR, Varicella: Age-appropriate doses.

Eye Examination: Required for first-time enrollment in Illinois schools.

5th graders can enroll in an optional Candor Puberty education which requires permission from parents.



 

6th

Physical Examination: Must be completed within one year prior to enrollment.

Immunizations:

  • Tdap: One dose at age 11 or older.

  • Meningococcal Conjugate Vaccine (MCV4): One dose at age 11 or older.

Eye Examination: Required for first-time enrollment in Illinois schools.



 

7th, 8th

Physical Examination: Required for first-time enrollment in Illinois schools.

Immunizations:

  • Tdap: One dose at age 11 or older.

  • MCV4: One dose at age 11 or older.

Eye Examination: Required for first-time enrollment in Illinois schools.

 

 Dental Examination Requirements:

  • Who Needs It: All students in Kindergarten, 2nd Grade, and 6th Grade. 

  • Deadline: Must be completed by a licensed dentist before May 15th of the current school year. 

  • Please ask about our free dental program offered.

 

Eye Examination Requirements:

  • Who Needs It: All students entering PK, Kindergarten, 2nd , 8th ,Special Ed, or enrolling in an Illinois school for the first time.

  • Deadline: Proof of examination must be submitted by October 15 of the school year.

  • Performed By: Licensed optometrist or medical doctor.

 

Mandatory Exclusion:

  • The State of Illinois Certificate of Child’s Health Examination form is due October 15th, of the current school year. Your child will be excluded from school on October 15th of the current school year until the forms have been completed and turned in a signed letter or form from your doctor indicating your child has a scheduled appointment for a health examination and will receive any needed immunizations.

 

Religious exemption:

  • A statement from the MD, DO, APN or PA indicating that an immunization is medically contraindicated is acceptable and needs to be attached to the student’s physical exam form. In case of a disease outbreak, the student must be excluded from school for 21 days or more mandated by the Illinois Department of Public Health. Religious exemption: A refused immunization requires an Illinois Certificate of Religious Exemption and/or Examinations Form completed by the parent and MD, DO, APN or PA for the current school year.
